A rare cause of shock: rabies vaccine induced Kounis syndrome

Introduction


Kounis Syndrome is a not unusual condition in daily practice. Its wideness of triggers is usually neglected. The objective of this clinical case report is to highlight rabies vaccine, a usual treatment for animal scratch, could potentially become the stimulator.

Clinical Case


A 56-year-old male demonstrated signs and symptoms of diarrhea, skin rashes and apathy was transferred to our facility after a rabies vaccine due to abnormal surface electrocardiogram and elevated serum cardiac markers. A diagnosis of myocardial infarction was made. However, the left and right selective coronary angiography was normal. The patient was treated with dexamethasone, nitroglycerin, and aspirin, and was discharged home in a stable condition several days later. However, on the 15th day after discharge, the event recurred shortly after the patient received his second rabies vaccine. He recovered with dexamethasone and aspirin.

Discussion


The case report aims to bring Kounis Syndrome, a not unusual syndrome, to the forefront, that every physician concerned should keep in mind of its wideness of triggers and the possible fatal evolution.
