Learning Objectives:
- • Describe the high risk TAVR patient due to the impact of small annular size on patient prosthesis mismatch and thus long-term outcomes.
- • Using a case example, describe how to weigh benefits and harms for the patient with a small native annulus, including how to assess potential risks of a TAV in TAV in the future, versus a valve-in-valve procedure with a small prosthetic valve amenable to valve fracture if the annulus is not enlarged at time of surgery.
- • Communicate with the audience how small annular size at initial evaluation for AVR impacts shared decision making discussions.
